I should be picking up a Carabela bike this weekend that is fairly complete with the exception of some chrome trim. One thing I noticed missing attaches to the frame just below the swing arm.
Is it a spring loaded chain tensioner with a pulley?

Yep and it's facing the wrong way or rather its not tensioned
Shouldn't the tensioner idler pully be on the underside of the bottom run of chain?

You'd think so if the chain is meant to be lifted , but , these tensioners seem to 'push' the chain down to maintain tension .
Either way does the job as all that's needed is to help keep slap to a minimum .
