There are shifter peds (sachs and puch) but, they would be imported as small motorcycles (if at all). Mopeds by definition aren't manual.
You won't find many from the "golden era". Before the early 70's mopeds were treated the same as motorcycles so there were a few 50cc models with manual transmissions. At the same time pedals were not required either. When the legal definition of moped was created almost all states required automatic transmissions. Because of this very few were imported. The sears allstate is probably the most common in the US. The others are so uncommon its tough to come up with a list. There are a handfull of shifter sachs engine bikes around, motobecane d52's, honda ss50's, yamaha fs1e's and puch monzas.
safari with pedals and a 4 speed I have one usa bike
morini made a 3 speed shifter in both kickstart and pedal versions.
Peugeot had a lot, for example the SX5 and GL10, Motobecane made the D55 tt among others. They were a lot made in and around the mid 70's.
here's another one which is pretty similar to the 1974 D55 which is probably the sexiest shifter moped ever made (even though it was only designed to have a top speed of 28mph).

The Casal, any Casal I would say do not molest it and leave it stock. They are not the most plentiful mopeds out there! If you want a bike to blast get a Puch or Tomos and do it up instead.
If it doesn't blast it won't get ridden it has all the right ingredients to be a real blaster.
Tate, There was a guy in Hollywood, Fl. Who worked in a bicycle shop, named Kevin. He has one still today, and that bike in the early 80's left all of us in the dust! When your ped does 60+, and you add gears to the equation... you can't touch it. One of the most awesome peds ever made!

I like this thread. Anybody have a multi-speed Ped titled/tagged as a moped? I have seen Honda MB5's with moped titles from time to time, I guess chalk that up to some state employees ignorance?
