1) Make sure you're not missing the toothed spacer (#14) and it's in the right place on the shaft.
2) Make sure the lever arm shaft (#1) isn't twisted
3) Make sure the end cam (#5) is oriented in the right direction
4) Hope the last guy didn't put the wrong fluid in the tranny. If he used super slick oil the rubber will never grab the bell.
If it is arm/cam related you can just replace the side cover. Side covers come up on eBay all the time for NOIs. I even have a spare if you just need the arm assembly (#1-6)
